Strong opposition from community health clinics and the California Medical Association has shaped trader expectations for Proposition 44, the union-backed initiative requiring federally qualified health centers to allocate at least 90 percent of revenue to patient services. Clinics filed a federal lawsuit in April 2026 challenging the measure’s placement on the November ballot, while opposition fundraising has reached over $33 million compared to roughly $17 million in support. Opponents argue the spending mandate and enforcement provisions would reduce operational flexibility and raise overall care costs, prompting coordinated resistance ahead of the vote. These factors, combined with the measure’s qualification via signatures collected by SEIU-UHW, have produced the current 65.5 percent implied probability against passage.
